Maydic Terms of Service

Welcome to Maydic.

Maydic helps you securely access healthcare services, manage your health records, coordinate your care, and make healthcare payments.

By creating your account or using Maydic, you agree to these Terms.

Please also read our Privacy Policy, Data Stewardship & Health Record Policy, and other applicable MCC policies.

1. Eligibility

You may use Maydic if you:

• Provide accurate registration information;

• Complete required identity verification where applicable;

• Keep your account information up to date; and

• Comply with these Terms.

2. Your Account

You are responsible for:

• Protecting your login credentials;

• Keeping your contact information current;

• Reporting suspected unauthorized access promptly.

3. Using Maydic

You may use Maydic to:

• Connect with healthcare providers;

• Manage your Care Team;

• Access your health records;

• Pay consultation fees;

• Receive prescriptions, laboratory requests, imaging requests, and referrals;

• Use other healthcare services offered through MCC.

Healthcare professionals remain responsible for medical decisions and documentation.

4. Care Team & Health Record Access

You control which healthcare professionals may join your Care Team, except where access is required or permitted by law.

Health record access is governed by the Data Stewardship & Health Record Policy.

5. Payments

Certain services require payment.

Where applicable, consultation outputs and clinical orders may remain unavailable until required consultation fees have been paid unless otherwise permitted by law or released by the healthcare professional.

Payments made through GoodTurn or third-party payers remain subject to MCC's payment policies.

6. Acceptable Use

You agree not to:

• Impersonate another person;

• Provide false information;

• Attempt unauthorized access to accounts or records;

• Misuse MCC services;

• Interfere with platform security.

7. Privacy

Your information is handled in accordance with our:

• Privacy Policy

• Data Stewardship & Health Record Policy

• Security Policy

8. Suspension or Closure

You may close your account at any time.

MCC may suspend or terminate accounts that violate these Terms, threaten platform security, or are used for unlawful or fraudulent activities.

Health record retention remains subject to applicable law.

9. Changes

MCC may update these Terms from time to time. Material changes will be communicated where required by law.
